Steps:

1) install java using jdk-6u45-windows-x64.exe.

2) install weblogic using wls1036_generic.jar (must use generic installer for x64), sourced from wls1036_generic.jar_V29856-01.  Use java -jar ..<full path>/wls1036_generic.jar

3) configure weblogic to create default domain using config wizard

4) install oracle forms 11g using ofm_frmrpts_win_11.1.2.0.0_64_disk1_1of1

5) install jre using jre-7u80-windows-i586.exe

6) add host to trusted sites using javacpl.exe in C:\Program Files (x86)\Java\jre7\bin

7) in Form Builder, go to Edit->Preferences->Runtime->Web Browser Location, and set to C:\Program Files (x86)\Internet Explorer\iexplore.exe
